A child-centred transition resource that captures a young person’s voice, sensory needs, and nervous system preferences — so the adults receiving them can create safety, connection, and predictability from day one.
Transitions can be exciting — but for children with SEMH needs, sensory differences, or neurodivergent profiles, they can also be profoundly dysregulating. This passport supports children moving to a new class, year group, or school by helping them externalise their inner experience in a way that can be understood and acted on — not judged or overlooked.
Grounded in Polyvagal Theory, trauma-informed practice, and neurodivergent-affirming approaches, every page is flexible and accessible — with space for drawing, symbols, or scribing to suit different communication styles, and checklists throughout to support thinking and expression without pressure. A practical tool for SENCOs, pastoral leads, and transition coordinators who want to get it right for the children who find change hardest.
