Outline:
This resource has been designed to give you an overview of potential need that a child may have. If you are working with a child and the interventions are not effective – this could give you an insight into why. For example, if a child has high attachment needs – putting them in a small group intervention, will not work. It also gives a basic overview of progress, against interventions.
Objectives:
- To provide a tracking system to monitor social and emotional interventions
- To provide evidence of progress towards EHCP targets

This assessment tracker has been created, to give you a snap-shot of the holistic needs of the child. It is broken down into the foundational skills:
- Attachment
- Sensory
- Emotional
- Social Skills
- Self-esteem
It is NOT diagnostic, this is an early intervention assessment tool – so that the facilitator completing this, gets an insight into why a child maybe struggling to access the world around them. Once you can see needs of the child, then you can put interventions in place – with the family, to support them. Once completed, it will identify areas of weakness in a particular area, that an intervention can support.
